Pregnancy Rules

When you copulate with someone of your same species (exceptions listed below), roll a d%. If the result is 3 or lower (15 or lower if the female is a human or half-elf), the female in the couple conceives. Also, for divination purposes, at this time roll a d%. On a roll of 1–49, the child is female. On a roll of 50–99, the child is male. On a roll of 100, she conceives twins. Reroll twice to determine the sex of each child.

Humans can mate with other humans, elves, half-elves, tieflings, and aasimar. The child’s race (for stat and ability purposes—cosmetically they may share the appearance of both parents) is randomly determined (50% either race), except in the case of elf and human, in which case the resulting child is always a half-elf.

Pregnancy Effects

One month after conceiving, you will begin suffering the effects of first-trimester pregnancy, as listed below. The first trimester's duration is 2 months. Each following trimester’s symptoms are cumulative, and occur 3 months after the previous trimester begins.

First-Trimester Effects

  • You have disadvantage on Constitution saving throws
  • You have advantage on Perception checks related to smell (this is not negated by exhaustion levels)
  • You can only travel for up to 4 hours a day; any faster is treated as a forced march.
  • You require a 10-hour long rest to get rid of a level of exhaustion instead of 8.

Second-Trimester Effects

  • Your movement speed is reduced by 10 feet.
  • You have disadvantage on Dexterity checks and saving throws.

Third-Trimester Effects

  • If you roll less than a 10 on any Strength or Constitution check, you suffer 1 level of exhaustion.
  • You automatically fail Strength, Dexterity, and Constitution saving throws.

Baby’s Health

Starting at 2nd trimester, the baby becomes vulnerable to outside effects and may be endangered by them. An unborn baby has 3 death saving throws. After 3 failures due to the effects below, you miscarry at the end of your next long rest, unless a raise dead or similar spell is cast on the baby before then. A failure can be removed if you spend an entire trimester without suffering another death saving throw, or with one magical healing spell, which must target the baby and has no effect on the mother. The following effects can cause you to miscarry:

  • If you are brought to 0 hit points, the baby fails a death saving throw. If you fail a death saving throw while at 0 hit points, the baby fails one as well.
  • If you take an amount of damage greater than half of your hit point maximum, the baby fails a death saving throw.
  • If you take poison damage or falling damage, or if you are subject to the poisoned condition or fall prone, make a Constitution check. On a result of 9 or lower, the baby fails a death saving throw.

Delivery

After 8 months of pregnancy, the DM rolls a d4 to determine the number of weeks until delivery will take place. At a random time that week, you go into labor. While in labor, your movement speed is halved, your hit point maximum is halved, and you have disadvantage on all attack rolls and saving throws.

Each hour, you must make a Strength check. When the total amount rolled equals or exceeds 30, you give birth.

At this point, in order to deliver the baby, another creature (or you, with disadvantage) must make two Medicine checks with a DC of 8 + 2 for each death saving throw failures the child has.

  • On two successes, the baby is delivered safely without complications.
  • On one success, the baby is delivered, but you are brought to 0 hit points in the process.
  • On two failures, both the baby and you are brought to 0 hit points.

After delivery, you gain two levels of exhaustion, and all the effects of childbirth fade from you after 1d12 hours.
