## Injury and Pain
Where hitpoints represent your stamina and your ability to keep functioning in a fight, your injuries represents how broken and wounded you are. Injuries persist from day to day and cause pain from moment to moment. Where rest, resolve, and determination allow you to regain your stamina, overcoming injuries requires time, a knowledge of medicine, and the mercy of the gods.
## The Short of it: Injuries
#### Being Injured
When a creature takes a particularly severe blow, takes damage while incapacitated, or gains certain kinds of status conditions, it may receive an injury. Each injury has a location and an associated numerical value known as an Injury Score which determines its severity.
#### Massive Damage
When you take damage from a single source equal to or greater than half your hit point maximum, you gain an injury. Roll 1d6 plus an additional 1d6 for every 10 damage taken above half. You gain a random physical injury with a severity equal to the value rolled.
#### Pain
All injuries, regardless of type or severity, can cause pain when you attempt to do something related to the location (or description) of the injury.
When you try to perform an action or movement which could aggravate one or more of your injuries, your DM may have you roll a d20 to determine how much pain doing so generates. If you would already roll a d20 as part of your action or movement use that roll instead.
If you roll less than your injury's score, then you become overwhelmed with pain. Your speed becomes 0 and you are incapacitated until the end of the turn.
If you have multiple injuries that are aggrivated at once, you add their scores together.
#### Severity
The severity of an injury determines the Medicine DC required to treat it, the amount of time it takes for that injury to heal naturally, as well as the level of magical healing required to cure it.

#### Treating Injuries

- **Natural Healing.** An injury heals naturally after you complete a number of long rests as given by the Natural Healing column of the Injury Severity table.
- **Medicine.** If you are proficient in the Medicine skill, you can spend 1 hour during a short or long rest to attempt to reduce the severity of an injury by making a Medicine check against a DC equal to the injury's Injury Score. On a success, the injury's Injury Score is reduced by your proficiency bonus. You cannot treat injuries with Injury Scores higher than your total character level.
- **Magical Healing.** An injury is healed immediately by the application of a spell which restores hit points of a level greater than or equal to the level given in the Spell Level column of the Injury Severity table. For magic items such as healing potions that directly restore hit points but have no level, the amount of healing required to heal an injury is equal to the injury's Injury Score.
#### Kinds of Injuries

- **Physical.** Blows, burns, cuts, and other forms of physical harm can cause a character to receive a physical injury.
- **Mental.** Powerful illusions or effects that induce insanity can cause a character to receive a mental injury.
- **Emotional.** Failure, heartbreak, betrayal, stress, or having your ability scores reduced can cause an emotional injury.
- **Spiritual.** Curses, Hexes, and certain kinds of spells or abilities may cause a character to gain a spiritual injury.
#### Mental
Mental injuries are harder to heal than physical injuries. Medicine checks cannot reduce their severity.

- **Psychic Damage.** When you take Massive Damage from a source dealing psychic damage (see Massive Damage in the Physical Injuries section), the injury may be treated as a mental injury rather than a physical one.
#### Emotional
Emotional injuries are difficult to deal with. Neither medicine checks nor magical healing can reduce their severity.

- **Despair.** When you are overcome with pain by 5 or more, make a Charisma saving throw against a DC equal to the amount of pain you have. If you fail, you gain a special emotional injury called 'Despair' with an Injury Score of 1. Each time you fail this check and are overcome with pain, increase the score of this injury by 1.
#### Spiritual
In general, spiritual injuries are rare and extremely difficult to deal with, as neither medicine checks, nor natural healing, nor magical healing can reduce their severity.
You may need high level magic or to complete a quest to remove a spiritual injury. Talk to your DM to determine if any of your spells or abilities might inflict spiritual injuries.
